Meadow was built by Alivia Parrish — a former registered nurse, PCOS patient, and mom. Not a tech company. Not a prescription mill. A woman who found real answers and built a platform to help others find them too.

Meadow is a PCOS-specialized telehealth practice. We're new — built by a woman with PCOS who couldn't find a provider who actually dug into the root cause. We start with comprehensive hormone testing, including a DUTCH test, and build a personalized care plan from your actual results. For some women that includes GLP-1 medication. For others it doesn't. Either way you leave with a real plan.
We're honest about being new because you deserve honesty — especially from a healthcare company. But here's who's behind it: Alivia Parrish, our founder, spent years struggling with PCOS. Dismissed by providers, wasted money, no real answers. It wasn't until she found a specialist who took a root-cause approach — including comprehensive hormone testing — that she finally understood what was driving her symptoms and had a plan built around her own results.

As a former registered nurse, Alivia couldn't go back to a system that had failed her. The DUTCH test is a comprehensive at-home hormone panel that measures your sex hormones, cortisol patterns, androgen levels, and how your body processes estrogen. Standard blood work misses most of this. The DUTCH test is how we're able to identify what's actually driving your PCOS — rather than guessing based on symptoms alone. You collect at home and mail it back. Results come back within days of the lab receiving your sample.
Not at all. GLP-1 medication is one tool your provider might recommend — it's not the default and it's not required. Some Meadow members never take it. Your plan is built from your lab results, your symptoms, and your goals. The medication conversation only happens if your clinical picture points that way. We never prescribe before reviewing your labs.
The PCOS Root Cause Assessment is $1,000. It includes your provider visits, your lab review, and a written protocol built from your results. If you continue into the Meadow Program, that $1,000 is credited toward the program price of $5,400, payable in full or over 12 monthly payments.

Lab costs are billed separately, directly to the lab. If medication is prescribed, it is filled through your pharmacy and priced by them, not by us.
No — Meadow is direct pay. We don't bill insurance. Lab costs may be partially covered by your insurer, and medication coverage varies by plan. We'd encourage you to check with your insurer directly.
Simple — love your first visit or it's on us. If your first appointment doesn't meet your expectations, tell us within 7 days and we'll refund your assessment fee in full. No questions asked.

This covers the visit itself. Lab costs are paid directly to the lab and aren't included. The guarantee applies to the assessment, not to the 12-month program.
From getting started to receiving your personalized care plan is about four weeks. That covers your first provider appointment, lab kit shipping and collection, lab processing, and your results review appointment. It takes time because we're actually looking at the full picture — not rushing to prescribe.
The assessment is a one-time purchase with a 7-day guarantee. The Meadow Program is a 12-month commitment, whether you pay in full or across 12 monthly payments.

We built it that way on purpose. Meaningful metabolic change takes a year, and a program you can abandon in month two isn't one.

Meadow is currently available to patients who live in Oregon or Washington. Everything is 100% online — no in-person visits required.

One thing worth knowing: because your provider is licensed in Oregon and Washington, you need to be physically located in one of those states during each appointment. If you are traveling, we will reschedule for when you are back. Your provider will confirm your location at the start of every visit.
